Account Maintenance Forms
- Authorization for Electronic Funds Transfer (EFT). Use this form to have monthly premium payments automatically deducted from your checking or savings account.
- Cancellation Request Form. Use this form to cancel term life, accidental death, children’s and/or dependent coverage for an in force account. This form can also be used to cancel an application for a new term life insurance account that is not yet in force.
- Name and Address Change Request. Use this form to process a name change for the insured and/or to change the mailing address for your life and/or Accidental insurance account. Do not use this form to change the ownership of your life insurance.
