Account Maintenance Forms

• Authorization for Electronic Funds Transfer (EFT). Use this form to have monthly premium payments automatically deducted from your checking or savings account.
• Beneficiary Change Request. Use this form to change the beneficiary designation for your life and/or accidental death insurance account. Do not use this form to change ownership of your account.
• Collateral Assignment. Use this form to designate the death benefit of your life insurance to a creditor as security for a loan. Call to obtain the form.
• Collateral Assignment Release. Use this form to release an existing collateral assignment for your life insurance account. Call to obtain the form.
• Name and Address Change Request. Use this form to process a name change for the insured and/or to change the mailing address for your life and/or Accidental insurance account. Do not use this form to change the ownership of your life insurance.
• Transfer of Ownership and Beneficiary Designation and Agreement Regarding Contributions. Use these forms to transfer ownership of your life and/or accidental death insurance.
