Permanent Medical Insurance for NJ Residents

Monthly premiums are based on your age, gender, product choice and NJ county of residency.

*Note: you must indicate your method of payment on page 4 of the application (Letter I) and include a check, payable to Horizon BC/BS NJ or a credit card number, for the first month’s payment. Your application cannot be processed if the first month’s payment is not included with your application.
