As a result of the Covid-19 health crisis, it may be difficult to obtain the lab work and paramedical exams that are typically required to underwrite an application. So we have temporary accommodations that may enable you to be underwritten without having to visit an unfamiliar lab for lab work or a paramedical exam.
- You may arrange to submit current labs and medical reports from your own doctor. If you choose to do so, please have your doctor complete this form. Upon completion of the form submit it here.
- MetLife will review your recently-submitted lab results and we will contact you if further information is needed.
- If you cannot provide current information yet wish to obtain coverage now, you may be able to secure a limited amount of insurance without an exam or labs. MetLife will allow you six months to complete underwriting requirements, at which time you can request an increase.
- All applications will be reviewed on a case-by-case basis, so additional underwriting exceptions may be made based on the information available.

During the Covid-19 health pandemic, some information typically used to underwrite applications, such as lab work and paramedical exams, has been unavailable.
If you have been approved for less coverage than you applied for or were not offered the best possible rates:
- MetLife will reevaluate your application and requested coverage amount once you are able to provide the necessary lab work and paramedical exams. Please contact us.
- If you cannot provide additional evidence now, we can assist you in requesting an increase and consideration for Preferred Plus rates at a later date.
To activate your coverage and continue insurance, premiums must be paid on time. Payment options:
- Monthly Electronic Funds Transfer. To activate coverage and begin EFT, click here.
- Semi-Annual Bills Payable by Check. To begin, mail us (a) a check for your initial semi-annual payment and (b) the top portion of your premium notice. Thereafter, we will send semi-annual notices to you. Except for your initial payment, each bill includes a service fee, which is currently $6.
- Ad Hoc by Credit Card. Single semi-annual or annual payments can be made by credit card by calling us.
After you make your initial payment, we will send your Certificate of Insurance.
